Acceptance Criteria for Funded Diabetic Foot Care.
Referrals for people to the Community Diabetes Foot Ulcer Prevention Service will be accepted if:
1. the person has diabetes, And 2. is enrolled in a Canterbury PHO general practice, AND3. a completed ERMS referral has been received from a general practitioner or practice nurse on the approved ERMS referral form, AND
4. the person has at least one of the following criteria; Loss of protection sensation (LOPS)Peripheral Arterial Disease (PAD)Previous Ulceration Previous Amputation
5. AND one or more of the following; End stage renal failureMāori or Pacific ethnicityA significant podiatry need;Excess callus with foot deformityPre-ulcerative lesionVery thick dystrophic toenails, involuted nails, mycotic nailsFoot pain
Before being seen by a podiatrist, a referral from a GP or Nurse must be in place and has to be accepted against this criteria. Unfortunately, some people with diabetes will not meet this criteria and will have to fund their own foot care.
